In accordance with the Act. 122/2013 Z.z. privacy (mentioned below as „ZOOOÚ) the person to whom it concerns grants consent to companies Consilium Consulting, s.r.o., Company ID: 35 946 504 that is registered in Commercial Register of the District Court in Bratislava I, in sec.: Sro, Entry No. 36891/B and Consilium Business Services, s.r.o., Company ID: 35 973 781 that is registered in Commercial Register of the District Court in Bratislava I, in sec.: Sro, Entry No. 39009/B both having their registered office in Bratislava, Štefanovičova 18 (mentioned below as „Operator“ and in common as „Operators“), to the processing of personal data provided electronically and/or in writing particularly to the extent of name, date of birth, permanent address, title, education attainment and practical experience and contact data as telephone number or email address (below mentioned as „personal data“). I agree to possible adding of other personal data necessary to fulfill the purpose of other processing.
The purpose of processing personal data is recruitment, temporary staffing for clients and processing of payroll over a period of 3 years. For that purpose I undertake to announce any change in provided personal data to any of the Operators within 14 days of the change with the fact that the Operator is entitled to announce the changes to other Operators.
The person to whom it concerns gives consent to the Operator to provide personal data from information system to third parties solely in the matter of processing personal data. I also agree with processing my personal data to the potential employers from countries of European Union.
In accordance with the Act § 28 Z.z. privacy the person to whom it concerns has the right upon written request to require from the Operators the following:
a. whether or not the personal data are processed,
b. in the generally intelligible form information regarding the processing of personal data in the information system,
c. in the generally intelligible form accurate in source information from which the Operators gain the personal data for processing,
d. in the generally intelligible form the list of the personal data which are processed,
e. correction or liquidation of the incorrect, incomplete or out of date personal data which are processed,
f. liquidation of the personal data if the purpose of processing was ended; if the subject of processing are official documents including personal data, the person can require returning the data,
g. liquidation of the personal data which are processed if the law has been violated,
h. blocking of the personal data due to withdrawal of consent before the expiry of the period of validity.
The person to whom it concerns has the right to ask any of the Operators to liquidate his/her personal data anytime. The Operator will liquidate the personal data without undue delay also if the personal data are incorrect or out of date.

By sending an email to the Operator I hereby confirm that the data which I had provided are truthful and accurate. I am aware that the Operator reserves the right not to accept my job application or to discard my personal data from information system in case of incorrectness of my personal data provided.
